Position Summary
We are seeking a dedicated Registered Nurse and Licensed Practical Nurse to provide exceptional in-home skilled nursing care to medically complex patients. You will serve as a vital link between families and the healthcare system, ensuring clinical excellence and family stability.
Primary Responsibilities
- Provide skilled nursing care for ventilator and tracheostomy-dependent patients.
- Develop and implement individualized care plans in collaboration with physicians.
- Monitor patient status and manage complex medical equipment at home.
- Educate family caregivers on patient care and emergency protocols.
- Advocate for patients' needs within the healthcare network.
Qualifications
- Active Registered Nurse (RN) license in the state of practice.
- Current BLS/CPR certification.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
Preferred Experience
- Experience in home health or ICU/Critical Care settings.
- Knowledge of ventilator and tracheostomy management.
- Experience working with medically complex pediatric or adult populations.
